Primary Care Management

Your primary care team will be responsible for organising your annual diabetes reviews and interim reviews as needed, and planning your care with you.

They will be well supported by the Community Diabetes Team in terms of easy access to telephone and email advice, as well as being able to organise patient support in the community.

Primary care teams will also have education and mentoring sessions with the Community Diabetes Team to ensure on-going development of skills and knowledge and the delivery of a high quality service.
