Your Diabetes

Community Diabetes Team

The Community Diabetes Team (CDT) is to help provide diabetes care closer to peoples homes.

Traditionally people with diabetes who needed specialist help needed to go to the hospital to receive this.

This often would be for decisions on some forms of treatment or advice regarding complications related to diabetes.

Some people will still need complex care provision and this will be provided by the hospital team at York hospital (see model of care - level 3 and the services section for more details) but many will have the benefit of receiving the specialist advice but at their practice clinics, community hospital or by telephone.

This will be through the CDT staff who will help the practice staff with providing level 2 care (see model of care for more detail regarding level 2 care).

The CDT staff are primarily the Diabetes Nurse Specialists and dietitians who are a part of the York Hospital specialist diabetes team.

They are supported by the Diabetes Consultants.
